INSURANCE

While I do not directly bill insurance, both psychotherapy and neuropsychological evaluation (after neurological illness or accident) may be covered in full or in part by your health insurance or employee benefit plan. I will provide you with the information needed for you to communicate with your insurance company for possible reimbursement. Please contact your insurance company for more information prior to your appointment and check your coverage carefully by asking the following questions:

 

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

 

SLIDING SCALE

While I don’t accept insurance, I do provide reduced fee services on a limited basis. Please discuss this with me during the initial phone interview.

 

PAYMENT

Cash, check and most major credit cards accepted for payment.

 

CANCELLATION

If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the cost of the session.
